Breathing trainers use the principles of resistance training to progressively overload the breathing muscles to improve their strength and stamina. There are three main ways of generating this resistance for respiratory muscle strength training:
1. Pressure Threshold Loading – the most widely studied and effective method
Pressure Threshold Loading is the most commonly used, researched and validated method. It provides a consistent resistance load that the user has to overcome with each breath during training. The mechanical POWERbreathe Plus (IMT) and POWERbreathe EX1 (EMT) use Pressure Threshold Loading. A precisely calibrated spring-loaded valve delivers the loading to guarantee accurate and repeatable training loads. We developed the POWERbreathe Smart Adaptor for use with these mechanical POWERbreathe devices.

The POWERbreathe EX1 is designed to train the muscles responsible for breathing out. Our EMST devices create resistance as you exhale to develop expiratory muscle strength important for coughing, swallowing, voice, airway protection and breath control for singing. Expiratory muscle strength training is also used across a range of clinical settings, including rehabilitation following a stroke and support for people with Parkinson’s disease.
With multiple resistance options available (the widest range available), users can select an appropriate starting level and increase the resistance on the EMST device as their expiratory muscle strength improves. Our handheld device is simply to use at home.Â
Users have the option to take their expiratory muscle training to the next level by pairing the POWERbreathe EX1 with the POWERbreathe Smart Adaptor and ActiBreathe App. Use the Smart Adaptor to perform a lung strength test. The app provides real-time feedback and personalized training programs. It can recommend a suitable training level and provide reminders to help you stay consistent with your training routine.
It only takes a few minutes to use the POWERbreathe EX1. You can use the device when standing or sitting, as long as the device is upright and the air inlet at the base remains uncovered. Take a full breath in, place the mouthpiece between your lips and exhale through the EMST device with as much speed and force as you comfortably can. Continue to do this until you have emptied as much air from your lungs as possible. After a short pause, you can repeat the process. We recommend performing five exhalations per set, five sets per day, for five days. After that time, review your resistance level as you may be able to increase the load.

Medium Resistance is suited to users with moderate to high fitness levels. The Heavy Resistance device is only intended for experienced users who have progressed to the top level of a Medium device. The POWERbreathe Wellness Plus offers the lowest and lightest starting ‘load’ (resistance) of all our devices (except for the electronic K-Series), and can be used by medical professionals to complement pharmacological treatments for conditions that IMST has been shown to be of valuable, including COPD and asthma, as well as by patients at home.
It is really easy to use the POWERbreathe Plus inspiratory muscle trainer. Breathe out as much as you can to empty your lungs. Put the mouthpiece in your mouth and take a strong, fast breath in through your mouth. It should feel a little bit harder to breathe than it normally would without the IMST breathing device. Train at a load that allows you to complete 30 breaths and repeat this morning and night. Increase the load as your training progresses. 2. Passive Flow Resistive Loading – a simpler and less precise method
In contrast to the precise training that Pressure Threshold Loading delivers, mechanical passive flow resistance breathing trainers are less accurate. This is because they depend on how much effort the user puts into their training, meaning the resistance can vary. This simpler method of loading is delivered via different-sized orifices. Due to the passive nature of the training this makes the training load unreliable and unrepeatable.
3. Dynamically Adjusted Tapered Flow Resistance Loading – the most advanced method
POWERbreathe developed dynamically adjusted, tapered flow resistance loading for the electronic K-Series of Inspiratory Muscle Training devices in 2010. We developed this method to allow adjustments within and between inspiratory breaths, making training more comfortable and tailored. As a result, this more versatile training method helps users maximize their efforts for breathing improvements all round.

The Smart Adaptor is not compatible with the POWERbreathe K-Series. This is because K-Series devices already offer guidance, feedback, analysis and additional training options.
3. Independent IMT & EMT for Better Results
With the mechanical POWERbreathe devices, you can train your inspiratory (IMT) and expiratory (EMT) muscles separately. The POWERbreathe Smart Adaptor and ActiBreathe® App offer guided inspiratory and expiratory muscle training independently. This allows you to focus on each type of training at different times. By training each breathing phase independently, you will achieve optimal performance gains. This is in contrast to other breathing training devices that combine both types of training in the same breath cycle, which can be overly challenging and less effective. This is because the resistance load placed on your in-breath and out-breath is too difficult when performed concurrently. This makes training uncomfortable and too much of a challenge, making it difficult to achieve maximal effort.
