Performing these breathing exercises daily can boost well-being, because dementia can affect respiratory coordination and breathing patterns.

People living with dementia may experience shallow breathing or signs of breathlessness during basic activities such as walking around the house or tidying. This guide will explain why some individuals with cognitive decline may experience these problems and how they may change over time. Breathing problems with vascular dementia are common, but with support from carers, individuals can feel more comfortable and have increased breathing awareness.

Neuroprotective Benefit: Inspiratory Muscle Strength Training (IMT / IMST) has not been tested for cognitive outcomes but can improve respiratory function in individuals with neurodegenerative disease, and may mitigate vascular-related dementia risk factors.

What is the Connection Between Dementia and Breathing?

Dementia and breathing are connected in more ways than one. Unfortunately, it’s a compounding problem. Poor breathing can accelerate cognitive decline because less oxygen reaches the brain. Then, as dementia advances, an individual’s ability to control respiration worsens, which has a knock-on effect.

Breathing relies on several organs and body systems working together. This includes the brain, nervous system, respiratory muscles and airways as well as posture. The diaphragm and other respiratory muscles need to be coordinated to maximise oxygen intake. If a person becomes less active and finds it difficult to follow instructions, then breathing patterns are likely to change.

Some people with dementia may start to breathe more shallowly. Some may not be able to explain that they are short of breath. Breathlessness can be caused by many different conditions, including chest infections, asthma, COPD, heart problems, anxiety, low fitness levels or medication side effects. Breathlessness may not be a direct consequence of dementia, but if carers, friends or relatives notice any worsening or unexplained breathing difficulties, they should speak with a GP or healthcare professional.

Vascular Dementia and Breathing Problems

Vascular dementia is caused by reduced blood flow to the brain. The symptoms of vascular dementia depend on which part of the brain is affected by these changes. Some patients experience difficulty planning and concentrating on certain tasks. It can affect everyday activities, such as getting dressed and eating.

The person suffering from vascular dementia may also develop physical symptoms. For example, a person with dementia may:

Sit in a more rounded or slumped posture

Move less during the day

Become weaker through reduced activity

Find it harder to coordinate movement and breathing

Feel more breathless during everyday tasks

Become anxious when they cannot understand or explain their symptoms

Breathing properly is essential for maintaining optimal health and well-being. Breathing training can help promote a proper breathing technique and allow the body to function effectively and efficiently.

Considerations to Make Before Starting Breathing Exercises for Dementia

The best breathing exercises for people with dementia are ones that can be managed effectively and ideally, independently. Depending on the stage of dementia, an individual may struggle to follow specific instructions. It’s important to note that a breathing exercise for one dementia patient may work really well and not so well for another. The aim is not to force deep breathing. The aim is to support easier, calmer and more comfortable breathing.

Before starting any breathing exercises, make sure the person is comfortable. It’s best to choose a quiet and airy environment. The person with dementia may find it easier to follow a demonstration of the breathing exercise rather than verbal instructions. Additional tips to make the process easier include:

Sit beside the person rather than standing over them

Use a calm voice

Keep each exercise short

Stop if they become tired, dizzy or upset

Avoid making the session feel like a test

Use the same routine each time

Choose a time of day when they are usually settled

Simple Breathing Exercises for People Living With Dementia

Breathing exercises for people living with dementia include supported upright breathing, pursed-lip breathing, Buteyko and resonant breathing. It’s probably best to keep the breathing exercises short. It may be more advantageous to do several short sessions rather than one long session where the individual loses concentration. This may be more effective in supporting vascular dementia breathing problems.

Supported Upright Breathing

Supported upright breathing is a good starting point for patients with dementia. It will help improve posture. To start this breathing exercise, ask the person to sit in a comfortable chair with both feet on the floor. They need to be sitting upright. Try to get the individual to relax their shoulders. If the patient is finding it hard, they may benefit from placing a cushion behind their lower back, as this will provide additional support.

The patient doesn’t need to do anything different to their breathing. Encourage them to breathe normally for a few moments. The idea is to train the individual to open up their chest and reduce slumping. This makes it easier for them to move the diaphragm and other respiratory muscles.

Pursed-Lip Breathing

Pursed-lip breathing should end up slowing down a person’s breathing pattern. To start the exercise, ask the person to breathe in (inhale) slowly through their nose for two seconds. They should keep their mouth closed during the inhale. Then, ask them to breathe out (exhale) through pursed lips, slowly, for four seconds. This should allow the individual to gain more control over their breathing.

Resonant Breathing

Resonant breathing is more complicated compared to other breathing techniques. Due to cognitive decline, it will work best to support dementia and breathing in the early stages of diagnosis. Carers may need to tweak it slightly for each individual to make it easier for them to follow. Resonant breathing should help patients slow their breathing pattern and feel more relaxed.

The person needs to sit comfortably in an upright position with their shoulders relaxed. They should breathe in gently through the nose for a slow count of three. Then breathe out slowly through the mouth for a slow count of four. The aim is not to force deep breaths, but to create a calm, steady rhythm. Start with three to five breaths and stop if the person becomes tired, dizzy, unsettled or confused.

Is POWERbreathe Suitable for People with Dementia?

POWERbreathe devices may be suitable for some people who are living with dementia. POWERbreathe inspiratory muscle training (IMT) is a clinically proven method of training the inspiratory muscles, including the diaphragm, to become stronger and resistant to fatigue.

In the early stages of dementia, inspiratory muscle training may be a structured way to support breathing muscle strength. However, patients in the later stages of dementia may become confused or anxious when using a device. In that case, simple carer breathing exercises may be more effective and appropriate.

Research highlighted in the New Scientist (2025) discusses a connection between high blood pressure as a risk factor for dementia. It states how lowering blood pressure can reduce the risk of dementia and cognitive impairment. Whilst IMST has not been formally tested in dementia patients, based on the ability of IMST to impact cardiovascular risk factors common to both cardiovascular disease and vascular dementia, it could potentially help reduce the risk of cognitive impairment related to vascular dysfunction. Findings from the landmark 2021 study conducted at The University of Colorado demonstrated that in 6 weeks, IMST using POWERbreathe can reduce systolic blood pressure as much as some medications.

“High-resistance IMST is a safe, highly adherable lifestyle intervention for improving blood pressure and endothelial function in midlife/older adults with above-normal initial systolic blood pressure”

Craighead et al.
